Our Schools:

The self-funded medical Plan administered by Allegiance Benefit Plan Management through the Cigna Network  includes two plans to choose from. The District also subscribes to an Employee Assistance Program through Alliance Work Partners that is offered at no cost to all employees and members of their households. The District provides $15,000 life insurance to all employees on the health plan and $50,000 life insurance from MetLife Insurance to employees who choose the hospital indemnity or alternate plan. Other voluntary benefits include self-funded dental plans administered by Allegiance Benefit Plan Management through the Cigna Network as well as a reduced fee for service plan from QCD; a long-term disability plan offered by UNUM; a vision plan by Vision Service Plan; and personal sickness indemnity, personal accident/disability, income protection, cancer, intensive care, hospital confinement and recovery plans offered by AFLAC. Employees have the option to participate in an Unreimbursed Medical account as well as a Dependent Care Account using pre-taxed contributions.

District Contributions
Medical Benefit $225 per month/$2,700 annual contribution
Life Insurance $15,000 term life with Medical Plan / or $50,000 term life with Alternate Plan 
 
Medical Benefits (click here for premiums) The District provides medical benefits with Allegiance Benefit Plan Management through the Cigna Network for full time employees who work 25 hours a week or more. There are three plan options from which to choose. They include:
 
 
  • Sample Insurance Card
  • This plan has no deductible and with a 30% co-insurance for in-network physician office visits
  • Physician Office visits are paid at 70% within the Cigna Network
  • You are not required to see a network physician; however, use of in network physicians is more cost-effective.
  • All in-network hospital facility charges are paid at 70%
  • Outpatient surgery is paid at 70%
  • Emergency Room visits are subject to a $200 emergency co-pay and then services are paid at 70%.   
  • Chiropractic Services are limited to 20 visits per year
  • There is an annual out-of-pocket maximum of $4,000 individual/$8,000 family
  • Maximum out of pocket for individual or family is met hospital services are paid at 100% ongoing visits to the physician, laboratory and diagnostic treatment such as x-rays are subjec to the co-insurance based on the services provided
  • All non-network charges are paid at 50% and do not accumulate to the out-of-pocket maximum
  • Wellness benefeits pay at 100% in accordance with AMA guidelines.
Informed RX 
  • $100 per calendar year deductible (September 1st- August 31st)
  •  Prescription co-pays from a retail pharmacy for name brands are the greater of $35 or the greater of 30% (30-day supply)
  • Generic $5 or the greater of 30% co-insurance
  • Mail Order maintenance drug co-pays are $20 generic/brand (90-day supply) 
  • Speciality Drugs have a $125 co-pay
  • Medication that is covered over the counter such as antacids and allergy medications are not covered on the plan

  • This plan has a $1,500 per plan year
  • Individual deductible $50 per plan year waived for preventative dental
  • Family deductible (three family members) $150 per plan year
  • Preventative dental (deductible waived) 100% of reasonable and customary
  • Basic restorative 80% of reasonable and customary
  • Major dental 50% of reasonable and customary
  • Orthodontic 50% - dependent children up to age 19

Low Plan

  • This plan has a $1,500 per plan year
  • $0 Deductible
  • Preventative dental (deductible waived) 50% of reasonable and customary
  • Basic restorative 50% of reasonable and customary
  • Major dental 50% of reasonable and customary
