Will your child need medication at school?
You will need to complete the MEDICATION ADMINISTRATION FORM. This form will need to be signed by a doctor if medication is required for more than 10 doses or 10 days.
Does your child need to carry an inhaler?
The following form needs to be completed by you and your healthcare provider:
Asthma Self-Administration Form
Does your child have a severe allergy requiring a physician prescribed epinephrine auto-injector?

Does your child have diabetes?
Please contact your campus nurse to set up a meeting via phone or Zoom to discuss your student's needs for the coming school year.
Does your child have seizures?
Please contact your campus nurse to set up a meeting via phone or Zoom to discuss your student's needs for the coming school year.
Does your child require special medical procedures such as tube feedings, catheterization or other procedure while at school?
Please have your medical provider fill out the appropriate order form below:
Specialized Medical Procedure Form (for procedures such as urinary catheterization)
Feeding Modification Form (if your student requires feeding through a g-tube)
Dietary Accommodation Request (required by ARAMARK if your student will purchase food or snacks in the cafeteria but has an allergy, religious restriction, or celiac disease which will determine which foods he/she can purchase).

Accident Insurance
Soon after the school year begins, parents will have the opportunity to purchase low-cost accident insurance that would help meet medical expenses in the event of injury to their child. Insurance forms are available at Student Accident Insurance Form. Paperwork to purchase this insurance must be returned directly to the insurance provider. Parents who have questions related to insurance should contact the insurance provider.
